探讨保留残余腱束与残端重建前交叉韧带后外侧束的临床疗效。

选取2016年5月至2020年12月36例前交叉韧带后外侧束损伤患者。其中男26例，女10例，运动伤32例，事故伤4例，年龄17~51岁。行关节镜下保留残余腱束与残端重建前交叉韧带后外侧束手术，术后随访进行国际膝关节文献委员会(IKDC)评分及Lysholm评分评估临床疗效，KT2000测量值评估膝关节稳定性。采用独立样本t检验分析数据结果。

36例患者均获随访，随访时间平均(19.2±2.5)个月。所有患者术后Lysholm评分、IKDC评分均高于术前，差异有统计学意义(t＝37.146、48.018，均为P<0.001)，KT2000差值术后平均为(1.6±0.8)mm小于术前(5.2±2.4)mm，差异有统计学意义(t＝8.538，P<0.001)。所有患者均未出现感染等并发症，关节稳定性及活动度恢复。

保留残余腱束与残端重建前交叉韧带后外侧束手术可以较好地恢复膝关节稳定性及功能，创伤小，允许患者早期功能锻炼，是一种疗效可靠的治疗方案。

To explore the clinical effect of reconstruction of posterolateral bundle of anterior cruciate ligamentwith preservation of the remnant fibers.

From May 2016 to December 2020, 36 patients with posterolateral bundle injury were treated. There were 26 males and 10 females, 32 sports injuries and four accident injuries. The age range was 17 to 51 years. The arthroscopic reconstruction of posterolateral bundle of anterior cruciate ligament with preservation of the remnant fibers were performed. After operation, International Knee Documentation Committee (IKDC) score and Lysholm score were used to evaluate the knee function and the KT2000 measurement was used to evaluate the knee stability. Independent t test was applied to analyze the data.

All 36 patients were followed up for(19.2±2.5)months on average. The Lysholm score and IKDC score of all patients after surgery were higher than those before surgery, the differences were statistically significant (t=37.146, 48.018, both P<0.001). The postoperative KT2000 were lower than those before surgery, the differences were statistically significant (t=8.538, P<0.001). No complication such as infection occurred in patients, and the stability and motion of the knee were recovered.

Reconstruction of posterolateral bundle of anterior cruciate ligament with preservation of the remnant fibers may be able to restore the stability and the function of the knee with less trauma and early rehabilitation, which is a reliable treatment.
