目的

探讨关节镜下盘状半月板损伤成形缝合术的中长期疗效。

方法

回顾性研究于2008年9月至2015年3月间在惠州市中心人民医院因膝关节盘状半月板损伤首次进行关节镜手术治疗的85例患者,男36例,女49例,年龄17~62岁,平均(32±10)岁,病程(1.5~22)个月,平均(7±4)个月。其中左膝52例,右膝33例,均为外侧盘状半月板损伤。按Watanabe分型:完全型49例,不完全型36例。对比术前、术后Lysholm评分及IKDC评分变化,了解术后Ikeuchi膝关节功能等级。其中对Lysholm评分及IKDC评分行配对样本t检验,P<0.05为差异具有统计学意义。

结果

随访时间2.5~8.3年,平均(4.7±1.3)年。手术后Lysholm评分(92±4)及IKDC评分(84±3)均优于手术前Lysholm评分(51±5)及IKDC评分(45±4),差异有统计学意义(t=5.39、4.78,P<0.01)。按照Ikeuchi膝关节功能评价方法,优68例,良12例,可5例,优良率94.1%。

结论

关节镜下成形、缝合手术治疗盘状半月板损伤,可以取得满意的中长期疗效,是1种理想的治疗方法。

Objective

To investigate the mid- and long-term efficacy of arthroscopic meniscoplasty technique in treatment of discoid meniscus injury.

Methods

Retrospective analysis of 85 cases of discoid meniscus injury treated with arthroscopic surgery from September 2008 to March 2015 in the Department of Orthopaedics, Central Hospital of Huizhou, were carried out. Among them, male patients were 36 cases, female patients were 49 cases.According to Watanabe classification, complete type were 49 cases, and incomplete type were 36 cases.The changes of preoperative and postoperative Lysholm scores and IKDC scores were recorded, and the postoperative Ikeuchi knee function level was reviewed. A paired sample t-test was performed on the Lysholm score and the IKDC score. P<0.05 was considered statistically significant.

Results

All the cases were followed up for(4.7±1.3)years; Lysholm scores increased from preoperative (51±5) to postoperative (92±4). IKDC scores increased from preoperative (45±4) to postoperative (84±3). Lysholm functional score and IKDC score were significantly improved compared with before treatment(t =5.39, 4.78, both P<0.01). Based on Ikeuchi’s grading, 68 cases showed excellent results, 12 cases good, five cases fair. The rate of excellent and good result was 94.1%.

Conclusion

Arthroscopic treatment is a preferred method for discoid meniscal injury.
