夏科氏关节病(Charcot′s arthropathy，CA)又称神经源性关节病，是以神经感觉和神经营养障碍为特点的破坏性骨关节病。CA可以发生在任何关节，发病的关节及部位与原发病相关。糖尿病患者中CA发病率为0.1%~0.5%^[1]，主要发生在踝关节。肩关节约占所有神经性关节病的5%，肘关节占3%~8%^[2,3]，而膝关节目前未见确切的发病率报道。全膝关节置换术(total knee arthroplasty，TKA)治疗夏科氏膝关节病(Charcot′s knee，CK)目前仍存在争议，由于疾病自身骨质破坏特点及早期假体设计及操作技的原因，CK曾被视为TKA的绝对禁忌证。但随着TKA技术及假体功能的不断优化，TKA治疗夏科氏膝关节病的相关报道如雨后春笋。然而，由于术后疗效不稳定，TKA实施仍具有一定的挑战性。故笔者拟报告3例TKA治疗CK的诊疗经过，分析其术前评估、假体选择、手术操作及术后固定与康复等步骤所存在的问题，以总结经验供临床诊治所参考。
