人工全膝关节置换术是治疗各种终末期膝骨关节疾病的标准术式,假体十年在位率超过90%。然而,仍有15%~20%的患者对治疗结果不满意,在这部分人群中,髌股关节异常引起的膝前痛及关节活动度减少可能是患者对治疗结果不满意的最重要原因。髌股关节异常主要包括髌骨轨迹异常、髌股关节复合体增厚以及髌股关节不稳等,而引起髌股关节异常的原因又可分为患者因素、手术因素以及假体因素,其中,手术因素与外科医生关系最为密切,是影响术后功能恢复结果的重点。

Total knee arthroplasty has been shown to be reliable at improving function in the patients with end-stage arthritis of the knee. The ten-year survivorship exceeds 90%. However, there are 15%-20% of patients who report dissatisfaction following primary TKA. In this crowd, anterior knee pain and decreased range of motion caused by patellofemoral abnormality may be the most important reason for patient′s dissatisfied with the treatment results. Patellofemoral abnormality mainly includes patellar maltracking, thickened patellofemoral composite and patellofemoral instability. In addition, the causes of patellofemoral abnormality include patient-related factors, surgical-related factors and prosthesis-related factors. Among them, surgical-related factors are vital for the postoperative functional recovery and most relevant to surgeons.
