髋腰综合征最早由Offierski在1983年提出,并将髋腰综合征主要分为3种类型。髋关节和脊柱由于在解剖位置邻近,生物力学上关系密切,导致髋关节和脊柱的退行性疾病在临床症状中有很多类似之处,如二者都可能会出现下腰部疼痛,给临床诊断与治疗带来许多困难。在诊断此类疾病时临床医师应把髋关节和脊柱作为一个密不可分的整体,查体时应仔细,避免因查体部位的遗漏导致的误诊,同时治疗也应兼顾髋关节与脊柱。髋腰综合征常见的手术治疗方式主要分为脊柱手术和髋关节手术,对于需要行这两种手术方式的髋腰综合征患者,何种手术应优先进行尚未有确切定论,目前的共识是应根据患者的病变部位及严重程度选择合适的手术方式和手术时机,及时做出干预,防止病情进一步进展。

Hip-Spine syndrome was first proposed by Offierski in 1983, and it was mainly divided into three types. Hip and spine are close in anatomical location and biomechanical relationship, which leads to many similarities in clinical symptoms of degenerative diseases of hip and spine. For example, both of them may have lower back pain, which brings many difficulties to clinical diagnosis and treatment. In the diagnosis of such diseases, clinicians should take the hip and spine as an inseparable whole. Physical examination should be careful to avoid misdiagnosis caused by the omission of the physical examination site. At the same time, the treatment should also take into account the hip and spine. Common surgical treatments for hip-spine syndrome are mainly divided into spinal surgery and hip surgery. For hip-spine syndrome patients requiring these two surgical methods, there is still no definitive conclusion on which surgery should be preferred. The current consensus is that appropriate surgical methods and timing should be selected according to the lesion site and severity of the patient, and timely intervention should be made to prevent further progression of the disease.
