Effects evaluation of theory of self-care in discharged elderly patients of knee osteoarthritis

Abstract:

Objective

To explore the effects of Orem theory of self-care in the discharged elderly patients of knee osteoarthritis.

Methods

From September 2013 to February 2017, 72 senile knee osteoarthritis patients in the department of orthopedics in Xiangyang Central Hospital who underwent hip replacement surgery were selected. Forty patients in the first orthopedics ward were the observation group, and 32 patients in the second orthopedics ward were the control group and were excluded people with intellectual disabilities. The control group were given routine discharge nursing, while the observation group were given graded extended care under the guidance of Orem theory of self-care. The nursing times in the two groups were lasted for three months, and the prognosis of two groups were recorded. The t test and chi-square test were used to analyze the relevant data.

Results

The compliance of the observation group and the control group were 100% and 87.5% respectively during the nursing period, and the observation group were significantly higher than the control group (χ2=5.294, P<0.05). The score of Hospital for Special Surgery(HSS) after nursing in the two groups were significantly higher than that before nursing, and the observation group after nursing weresignificantly higher than that of the control group(t=7.444, P<0.05). During the nursing period, the incidence of complications such as thromboembolism, infection and urinary retention was 10% in the observation group, while it was 40.6% in the control group. The observation group were significantly lower than the control group (χ2=9.224, P<0.05). The satisfaction of the observation group and the control group were 97.5% and 81.3% respectively, and the observation group was significantly higher than the control group (χ2=5.349, P<0.05).

Conclusion

The application of graded extended care under the guidance of Orem theory of self-care in the discharge patients of elderly knee osteoarthritis can improve the function of knee joint, improve the compliance of functional exercise, reduce the occurrence of complications, and improve patients' satisfaction with nursing work.
