Clinical effect of multiple joints replacements in patients with severe rheumatoid arthritis

Abstract:

Objective

To investigate the clinical effect of multiple joints replacement in patients with severe rheumatoid arthritis.

Methods

A total of 21 patients (47 hips and knees) with severe rheumatoid arthritis treated in the Department of Joint of the Second People’s Hospital of Shenzhen from January 2018 to December 2021 were selected. The clinical effects, X-ray and complications of the patients were analyzed. The quantitative data such as joint motion and function score index were analyzed by t test, and the complications were analyzed by descriptive analysis.

Results

The treatment of all the patients was successfully completed. During hospitalization, two patients were found deep venous thrombosis of lower extremities and one patient developed superficial wound infection. All the three patients recovered after conservative treatment. At three months after the last joint replacement, the Harris s hip core of the patients increased from (24±23) before operation to (89±10) after operation(t=11.97, P<0.01); the Hospital for Special Surgery knee score (HSS) increased from (29±16)to (86±7)( t=9.64, P<0.01); the flexion range of motion of hip increased from (41±12)° to (88±15)°(t=17.03, P<0.01), and the flexion range of motion of knee joint increased from (50±15)° to (95±12)°(t=21.39, P<0.01). The percentage of patients subjective satisfaction of grade Ⅲ-Ⅳ was 93.0%.

Conclusion

Total hip and knee replacements are effective methods for the treatment of severe rheumatoid arthritis, which can correct the deformity of the joint, increase the range of motion, improve joint function, improve patient satisfaction and quality of life.
