Total hip arthroplasty combined with autologous cancellous bone implantation for treatment of femoral neck fractures in elderly patients

Abstract:

Objective

To investigate the clinical efficacy of total hip arthroplasty combined with autologous cancellous bone implantation into femoral bone marrow cavity in the patients with femoral neck fracture.

Methods

A total of 120 patients with femoral neck fracture treated in the Department of Orthopedics of the First Affiliated Hospital of Xi′an Jiaotong University from January 2019 to June 2021 were enrolled. According to the treatment methods, the patients were divided into the control group (total hip arthroplasty) and the research group (total hip replacement combined with autologous cancellous bone implantation in femoral bone marrow cavity). The operation time, intraoperative blood loss, postoperative drainage, quality of life score and complication rate before and after the surgery were compared between the two groups by t test.Chi square test was used to compare the recovery of hip function one year after the surgery between the two groups.

Results

There was no statistically significant difference in operation time between the two groups (P>0.05), while the intraoperative blood loss and postoperative drainage were lower in the research group than those in the control group (t=20.234, 26.539, both P <0.001). The excellent rate of hip function recovery in the research group one year after surgery was higher than that in the control group (χ2=1.905, P<0.05); there was no statistically significant difference in the complication rate between the two groups (P>0.05). The postoperative scores of life quality, social, physiological and psychological functions in the research group were higher than those in the control group (t=5.788, 9.485, 10.967, 9.642, all P<0.001).

Conclusion

otal hip arthroplasty combined with autologous cancellous bone implantation in the femoral bone marrow cavity can effectively reduce intraoperative blood loss and postoperative drainage in the elderly patients with femoral neck fracture, improve the function of hip joint one year after surgery, and improve the overall quality of life, which is worthy of clinical research.
