Clinical observation of absorbable rod for treatment of osteochondral fracture of patella dislocation

Abstract:

Objective

To investigate the clinical effect of absorbable rod fixation in treatment of osteochondral fracture caused by traumatic patellar dislocation in adolescents after effective rehabilitation treatment.

Methods

A consective series of osteochondritis dissecans of patellar dislocation performed with the bioabsorbable pins fixtion were retrospectively studied. All the patients received surgical treatment for osteochondral fractures in the Second Affiliated Hospital of Inner Mongolia Medical University between 2012 and 2014. Inclusion criteria: patellar dislocation accompanied by osteochondral fracture, age less than 18 years. Exclusion criteria: osteochondral fracture caused by other reasons, and age over 18 years. All the patients received MRI examination before surgery. Fifteen patients met the inclusion criteria, including four males and 11 females. The average age was (14.6±1.8)years. Eleven cases osteochondritis dissecans happened in the patella, four cases osteochondritis dissecans occurred in the femur. All the osteochondral fractures adopted open reduction and absorbable rods fixation treatment, postoperative immobilization, physical therapy of the patients were unified treatment scheme. knee injury and osteoarthritis outcome score (KOOS), International knee Documentation Committee Knee evaluation form (IKDC), Tegner score were used to observe the clinical effect.

Results

Fifteen cases showed osseous healing, and all the patients after reasonable postoperative rehabilitation had no complication. The mean follow-up time was (2.4±0.7)years. The KOOS score(86.9±5.8), IKDC scores(88.7±6.1), Tegner score(5.9±1.6) at follow-up were improved comparing to the preoperative KOOS scores(71.2±6.2), IKDC scores(74.1±6.0) and Tegner scores(4.1±1.7); the differences were statistically significant (t=-8.6, -6.3, -7.1, all P<0.05).

Conclusion

Absorbable pins fixation treatment of osteochondral fracture in effective rehabilitation clinical curative effect is reliable, by the middle of the follow-up, has high bony healing rate, no secondary surgery patients.
